Access your Magnus Health SMR account by logging into our Online Portal and clicking “Health and Safety.” Once on the page, scroll down and click on the Magnus Health logo under “Access Magnus Health,” and you’ll automatically be logged in. New families will receive access to Magnus in mid-July through an email with instructions.
Parents must finish all sections in the Magnus Health Portal, either by filling in forms in Magnus, or by downloading, filling out, and then uploading forms to Magnus.

For children who do have medical needs, please make sure your Action Plan is completed, with a physician’s signature, along with any needed Prescription Medication Consent.
Your child needs an annual medical exam, which includes your child's height, weight, blood pressure, and results of both vision and hearing exams, to be performed by a licensed practitioner, (NY, NJ, CT Healthcare Provider) no later than a year from their previous exam (DOH utilizes the “date of exam” to determine compliance). Also, children entering Groups IIs–Vs need a lead risk assessment test to be performed by a licensed practitioner, no later than a year from their previous exam. Newly-dated print-ups of information from old exams are not acceptable.
Section 2164 of the Public Health Law, of the New York City Department of Health and Mental Hygiene, mandates that all students—children entering day care, nursery school, pre-school or pre-kindergarten, and kindergarten through grade 12—comply with immunizations requirements for school entrance and attendance. No. All information, including forms from family physicians, should be input directly into the Magnus system. Please do not pass anything to the front desk or School Nurse.
No medication, including over-the-counter medication, can be given without parental AND healthcare provider consent. Over-the-counter medication will be dispensed at the discretion of the School Nurse. The amount of medication dispensed is based on your child’s age and weight. All medication is to be dispensed in the Nurse’s office. Please do not give your child medication to be taken during the day—including cough drops.
Please advise the School Nurse of any injuries or new medical conditions that happen throughout the year, especially over the summer. A doctor’s note is required for any injury or new medical condition, along with instructions for when the child can resume activity, and the care and support they need while in school.
Make sure to update Emergency Contacts as necessary throughout the school year.
Please have EpiPens, asthma inhalers, and other medications needed in school available to the School Nurse on the first day of school. We require two (2) EpiPens and two (2) inhalers—one to be kept in the Health Office and the other in emergency bags kept in your child's classroom. Please bring ALL EpiPens, inhalers, and medication directly to the health office. Medications are NOT to be sent with students. Please ensure medications are labeled with your child’s name and Group.
You will receive alerts throughout the summer from Magnus Health until you complete your child’s record. Magnus will also send out alerts when records are approaching expiration dates and will continue to send out alerts until these requirements are fulfilled.
